About the role
Northeast Restorative Therapy Services is seeking a licensed Physical Therapist to evaluate and treat short-term rehab and long-term care residents in a skilled nursing and sub-acute rehabilitation setting at our partner facility in Riverhead, NY. You will lead the plan of care for residents recovering from orthopedic surgery, stroke, cardiac events, and general deconditioning, helping them return safely to their prior level of function.
This is a full-time position (35+ hours per week) with a predictable weekday schedule and occasional weekend rotation.
What you'll do
- Complete initial evaluations and establish individualized, evidence-based plans of care
- Deliver skilled physical therapy interventions for orthopedic, neurologic, cardiopulmonary, and deconditioned residents
- Establish measurable functional goals and progress residents toward safe discharge
- Supervise and delegate appropriately to Physical Therapist Assistants
- Document evaluations, daily notes, progress notes, and discharges in PointClickCare / NetHealth in compliance with Medicare and payer requirements
- Participate in interdisciplinary care-plan meetings and collaborate with nursing, OT, and Speech
- Recommend durable medical equipment and provide caregiver/family training
What we're looking for
- Degree in Physical Therapy from a CAPTE-accredited program (DPT preferred)
- Active New York State Physical Therapist license, or license/limited permit pending
- Current BLS/CPR certification
- SNF or sub-acute experience preferred; new graduates with strong clinical affiliations welcome
- Working knowledge of Medicare Part A/B documentation and PDPM a plus
